2. What Information Do We Collect?
We collect information at several different points on our Web site. Personal information is collected when using our online contact form, booking an appointment, or joining programs such as the OneMatch Stem Cell and Marrow Network, Partners for Life, and others. Individuals are free to use this Web site without participating in any electronic submission that requests your personal information, and thereby avoid providing identifiable personal information.
When you visit our Web site, our Web server automatically collects a limited amount of information essential for the operation and security of our Web site. Some of this information gathered (e.g., the date and time of visits, the pages viewed, time spent at our Web site, and the Web sites visited just before and just after our Web site) does not identify who you are. Other information, such as your Internet domain name or your Internet protocol address may identify you. The extent of personal identification may depend on the "naming standards" followed by your Internet Service Provider. In addition, our advertisers and ad serving companies may also use small pieces of code called "web beacons", "spotlights" or "clear gifs" to determine which advertisements and promotions users have seen and how users responded to them.
When you voluntarily send us electronic mail, we will keep a confidential record of this information so that we can respond to you.
We collect information that personally identifies you only when you provide it to us by filling in an online form. This information may include your email address, blood donor identification number, name, date of birth, postal address, and telephone number. You will also be required to provide personal health information when joining OneMatch Stem Cell and Marrow Network. All identifiable personal information that you provide, including your personal health information, will be kept confidential.

We do not share your identifiable personal information with third parties (except as described below), public bodies or individuals unless it is with your consent or as authorized by law. We may have third party service providers perform services relating to the operation and maintenance of our Web site or for the purpose of sending you information by way of an electronic newsletter or through regular mail ("Third Party Service Providers"). We may share your identifiable personal information with these Third Party Service Providers as is reasonably necessary for the purpose of providing their services to Canadian Blood Services. All Third Party Service Providers are required to sign a non-disclosure agreement with Canadian Blood Services to protect your identifiable personal information, and do not have the right to sell or share your personal information with other third parties.
